Fall Campout 2017
WHERE: Pataps Valley State Park McKeldin Area Youth Group Sites 1 and 2 Follow signs once at the park.
WHEN: September 29th, 30th, October 1st
COST: $25 (per family, payable by check or Square)

| REQUIRED FORMS:
Medical Forms:
- Every camper must download the 2014 BSA Medical form below.
- Please make a copy of your medical insurance card (front and back).
- Put the 3-4 pieces of paper in a standard envelope, seal it, and write the person's (or family's) name on the front.
* All medical forms are for the sole purpose of giving to the local hospital in the event of an emergency in which the parent is not available or incapacitated.
Medical Forms are now good for one year and will be saved until the end of the Scouting Year (May 2017) unless labeled to be returned or destroyed after the campout.
Activity Consent Form:
The activity consent and hold harmless agreement is a separate, less sensitive document. They must be completed for each activity and will be destroyed after the campout.
Payment and forms (medical & activity consent) must be dropped off at 7110 Newberry Drive by 9/25!
